2011 Matt Crowder Memorial Scholarship Application
Purpose
This scholarship was established to promote the same community focus and perseverance in sports, academics and life that Matt Crowder displayed during his lifetime. Scholarships will be awarded to two eligable seniors at each Andover high school. A $250 scholarship will be given to each student for use towards college tuition, fees or books. Winners will be notified when scholarships are awarded during the end of the school year at each high school.

Eligibility
Seniors who attend either Andover or Andover Central High School in Andover, Kansas during the 2011-2012 school year.

Criteria
1. Student athlete involved in one or more school sponsored sports at either Andover High School. (Team managers are eligable)
2. Maintain a 2.5 cumulative high school grade point average.
3. Student in good standing. (Administrator approval, no outstanding disciplinary actions).
4. Volunteered for activities outside of the school. This includes volunteer work for community organizations, tutoring, big brothers and big sisters, scouting, etc.
5. Volunteer at the 2011 Matt Crowder Memorial Andover Bowl.
6. Entering college in the summer or fall of 2012.
7. Applications must be submitted by March 1, 2012.

The Matt Crowder Memorial Andover Bowl is a post season junior football tournament open to all Mid-States junior football teams grades 3 through 6. This year's bowl will be held on November 4th, 5th and 6th in Andover Kansas.
All teams will play at least 2 games. All players will receive a custom medal for participating. Additional awards will be awarded to each player on the first and second place teams. The head coach on each team will also receive a team trophy for first and second place. The head coaches from each team in the championship game will award an MVP trophy to a player on the opposing team. Referees and bowl organizers will select one player from each championship game to receive the coveted sportsmanship award. This award is given to a player who showed good sportsmanship througout the game to both their team mates and the opposing team.
8 teams will be selected to play under the lights on opening night, Friday, November 4th*. *Schedule subject to change based on field availability.
Hot food and cold drinks will be available for sale at each of the event locations.
Purchase an event pass wrist band and get into every game, all weekend, for free.
2011 Matt Crowder Memorial Andover Bowl
Junior Football Tournament
Date: November 4th, 5th and 6th, 2011
Location: Andover, Kansas
Participants: All 3rd, 4th, 5th and 6th grade Jr. football teams in the Mid States Junior Football League.
Entry Fee: $195 per team before October 11th, 2011. $225 per team after October 11th, 2011. Deadline: All team entries and fees must be received by October 16th, 2011.
Tournament Details:
All Teams will play at least 2 games.
All games will be played using the Mid States Jr. Football League 2011 Rules.
Bracketing will be based on team record within the Mid States Jr. Football League.
*All 6th grade playoff games will be played at District Stadium with 4 referees on the field.
*All 5th grade playoff games will be played at South District Stadium with 4 referees on the field.
*Games will start at 6 pm Friday and 8 am on Saturday and Sunday.
Only the first 8 team entries will be accepted per division. REGISTER NOW!
All Participants must be able to provide proof of age and have a signed release form to participate.
Team rosters must match the Mid-States roster. No team padding!

Awards:
Participation medals for all players and cheerleaders.
Team and individual trophies for the championship and runner up teams.
MVP trophy for 1 player on each team in the championship games.
Sportsmanship trophy for 1 player in each championship game.

Additional Information:
No admission on 3rd grade field for those games played behind Middle School on Saturday.
All Stadium Games: $5 admission on Friday, $5 admission on Saturday, $5 admission on Sunday. WEEKEND PASS: $8 three day pass includes wrist band and program (while supplies last).

First Place Team: This award will be given to the team in each grade that wins the championship game. The team will be presented with the game ball to be signed and placed in the case. One award will be awarded per team.

Sportsmanship Each grade will have one sportsmanship trophy awarded after the championship game. This award will be given to a player in the championship games whom the bowl organizers and referees feel displayed the best sportsmanship during the bowl.

Participation Medal This medal will be given to all football players who participate in the bowl. Players on the first place teams will receive a gold version of the medal. Players on the second place team will receive a silver version of the medal. All other players and cheerleaders will receive a bronze version of the medal.

It was through tears that Matt Crowder called home that autumn evening and said, "Dad, they called me 'coach'." It had been his dream. So it was not a surprise that Sunday afternoon in November 2005, when he called home again - this time with hardly a voice left from yelling - to say, "Mom, we WON!!! I am standing on the football field with my team. We won the Newton Bowl! We are undefeated! I love you, mom."
 
Today, you have honored our son, Matt by your participation in this tournament. We cannot think of a more fitting tribute. Thank you.
 
To the players: Coach Crowder would tell you to give it all you've got. You can do it. Don't stop now. Be the best you can be. Don't let anyone tell you that you are too little, too short, or too slow...You can do it. You can do it in football...and you can do it in life.
 
To the coaches: You set the stage for a lifetime of achievement. Every child deserves a chance. Every child needs encouragement. Every child needs someone to look up to. Don't take it lightly that you might be that "someone".

To Matt's friends (and there are so many): Your friendship is so unique. What started out as a 7th grade team has turned into lifelong friends. Matt Crowder was blessed to have such friends. Thank you.

Hypertrophic Cardiomyopathy
Matt suffered from a type of heart disease known as Hypertrophic Cardiomyopathy (abbreviated HCM). You may not have heard of this type of heart disease. However, about 1 in 500 people are afflicted with HCM. HCM is predominantly a genetic disease, but many cases are not genetically linked. HCM is a disease that affects the heart muscle. The most consistent feature is excessive thickening of the walls of the heart muscle, primarily the septum. Some people call this enlarged heart. HCM is one of the silent type diseases. With no family history to help the physician, a typical health physical will not identify the disease. An echocardiogram (ultrasound of the heart) is normally required to identify HCM. With early detection a person can have a normal productive life. In Matt's case, there was no detection.

7,000 to 14,000 young people die from

Sudden Cardiac Death every YEAR (CDC statistic).

36% of these deaths are directly attributed to HCM.

1 in 500 people of the general population have HCM;

Scholarship Information
The original intent of this bowl was to raise money primarily for the Andover Junior Football program as well as the other Andover school football programs. We have established a memorial scholarship in Matt's name.
A scholarship has been established up for 2 high school seniors at each of the Andover high schools. A fixed amount will be donated per recipient towards their college education. We will select candidates who submit an application based on their good standing (no bad grades or other disciplinary issues), community service and volunteer status with the annual Andover Bowl.
All funding for scholarships will be taken from the proceeds generated at the annual bowl event.

Position Description
Runner: Must be an adult (18+) with a cell phone and available car. Assists staff and patrons on game day by running errands, emptying trash, running supplies between sites and assists with concessions.
Clock: Runs game clock and scoreboard during games.
Sales: Selling T-Shirts. Must be 13+ to handle money with parental supervision.

Andover Junior Sports Association - Mission Statement

The Andover Junior Sports Association's (AJSA) mission is to improve youth football programs in the city of Andover, Kansas. We do this by raising funds for the Andover Junior Football program, the Andover Middle School program and the Andover High School program. Our fund raising activities are directed at increasing the awareness and support for football in the Andover community.
The junior football program receives funds from our organization that are used to buy practice equipment, helmet decals and provide for an end of the year banquet for each of the players. In the event the City of Andover does not have the ability to provide a scholarship for a player who can't afford to play, AJSA will provide funds to help them meet the costs. The senior high and middle schools each receive an equal donation of equipment for use in their programs. In the past, this has included helmets, chin straps and other safety related equipment needed by the players. In addition, AJSA provides 2 scholarships to each high school for senior athletes who are in good standing and are planning on enrolling in college in the following year.
AJSA promotes community involvement and awareness by holding an annual junior football tournament. This event brings in over 2,500 junior football players, spectators and local volunteers. The event is held each year in honor of a former junior football coach who passed away. The community is encouraged to come out and watch the junior football players compete in a 2-3 day tournament that culminates in a championship game for each grade level.
